***This position is covered by the SSG Fox Suicide Prevention Grant Program, which runs from October 1, 2024 - Sept 30, 2025 with an anticipated renewal annually through Sept 30, 2027.***

Job Summary                                                                                                                                                                                    

Clear Path for Veterans, Inc. (CPV or Clear Path) is a community-based organization delivering world class, holistic programs and services to veterans, service members, and military-connected families without condition.  The SSG Fox Program Intake and Assessment Coordinator is responsible for conducting intakes to assess each Veteran for eligibility under the SSG Fox Suicide Prevention Grant (i.e., Patient Health Questionnaire, Generalized Self-Efficacy Scale, Interpersonal Support Evaluation List, Socio Economic Status, Warwick Edinburgh Mental Well Being Scale, Columbia Suicide Severity Rating Scale), and conducting “warm hand-off” referrals to the VA for mental health care and to a Peer Mentor for peer support services. The incumbent of this position may be required to coordinate with therapists, psychologists, and other health care professionals regarding veterans participating in SSG Fox SPGP programs and services.

 

Primary Functions                                                                                                                                                                           

  1. Assess Veterans for suicide risk using VA-approved clinical assessments and determine eligibility for SSG Fox SPGP services.
  2. Conduct warm hand-off to the VA for suicide prevention services, including, but not limited to evidence-based therapies, medication, substance use disorder treatment, and other supportive treatment options.
  3. Connect eligible Veterans to a SSG Fox Peer Mentor to receive Peer Support and connect family members of the Veteran (if applicable) to the SSG Fox Family Support Manager for support and resource coordination.
  4. Obtain appropriate assistance for veterans experiencing immediate crisis while participating in program-related activities and direct them to the VA and other community resources, as necessary.
  5. Completion of required administrative paperwork directly relating to the suicide prevention component of CPV programs and services.
  6. Using data collected from screening matrices to assist in program
  7. Act as a liaison for veteran clients and advocate for connection to community and/or national
  8. Provide training for staff performance/enhancement as requested related to the field of veteran
  9. Complete other related duties as

 

Education and Experience Requirements                                                                                                                                 

 

  1. Bachelors Degree in Social Work or Human Services required,
  2. Preferred 4 years work experience working with Veterans suffering from mental health issues.
  3. This position requires the ability to work a flexible schedule, including non-traditional hours as necessary to accommodate for special event attendance.

Job-related Physical Abilities needed in order to perform the job in a satisfactory manner                                      

 

May sit or stand for long periods of time. Manual dexterity sufficient for constant use of computer keyboard and other office equipment. Vision sufficient for frequent use of a computer monitor, developing and reading paperwork.
